Anderson Student Forms

UNM Dane Smith Hall Below are forms for current and potential Anderson BBA students. If you have questions, please contact the BBA Advisement Office.

  • Change of Concentration Form
    Use this form if you want to change your concentration or add a second concentration once you have been admitted to Anderson. When you have indicated your changes on the form and signed it, please drop it off at the front desk of the Advisement Center so that your degree plan can be updated in LoboWeb and noted in your file.
  • Concentration Course Approval Form
    Use this form if you have received permission from your faculty concentration advisor (see the worksheet for your concentration) to substitute a course not specified as part of your concentration curriculum. Once it is signed, please drop it off at the front desk of the Advisement Center so that the substitution can be noted in your file.
  • Course Override Request Form
  • BBA Core Curriculum Requirements ("Purple Sheet")
    BBA Applicants can use this form to determine how many course requirements and hourse have been met.
  • Degree Summary Form
    Use this form for verification of your degree progress for your employer or an internship or scholarship application. Most scholarship applications require you to project your remaining semesters and note your graduating semester. Once complete, please drop it off at the front desk of the Advisement Center for review by your advisor, usually within 24-48 hours. If you leave your contact information (email address and/or phone number), our front desk staff can notify you when the form is ready to be picked up.
  • External Minor Form
    This form is to be used by students currently in the BBA program who wish to declare a minor outside of Anderson. It must be signed by the advisor in your minor department and courses completed/in progress/remaining must be noted. Once complete, please drop it off at the front desk of the Advisement Center so that your minor can be added to your degree plan and noted in your file.

    Please note: If you are in another degree program and wish to declare a management minor, this needs to be addressed with an advisor in your college (Arts & Sciences, Engineering, Education, etc.)
